How do I apply for licensure as a Doctor of Oriental Medicine?

If you wish to apply, please go to the LICENSING link on our website and select the appropriate application. Complete the application and mail to the Board office. If you have questions please phone, send an e-mail, a letter, or fax to:

      If you are unable to download the application, please call or email the board and request an application be sent to your mailing address. The board employs their staff as independent contractors, the office is not routinely staffed for walk-in traffic or personally answered phone calls. When you call the board you will receive a voicemail message. Please leave a message and the Executive Director will return your call. If you need to make an appointment with the Executive Director, please phone or email.

        When do I have to send in my application?

        Your application must be received 3-6 months prior to the next examination date. Exam dates are in June and December and possibly in March/April and Sept/October. The approval process will be dependent upon a complete application and if the applicant complies with the Oriental Medicine Board’s licensure requirements.

          How many licenses have been issued by the Board?

          If I have a complaint concerning the practice of a Doctor of Oriental Medicine, how would I file it with the Board?

          The Board accepts any written, signed complaint. There is a Complaint form in the FORMS link on our website. Please tell in your own words, in four paragraphs Who, What, When, and Why/How. You must name the involved parties and the circumstances of the complaint. You must sign the letter and be willing to participate in a Board investigation. Please mail this complaint to the Board at the address above.

            It then will be forwarded to our Deputy Attorney General and the Board members for further review and investigation.

              Where and when are the Board's meeting?

              The Board meets via Zoom. Meetings are held several times each year. Please check our home page for the next meeting date. If you would like to be put on our Agenda Mailing list, please phone, send an e-mail, a letter, or fax and give us your name and email address so we may put you on our list to notify you of the next meeting.
