Fingerprint Background Waiver - IMPORTANT INSTRUCTIONS!

1. Please make sure that you complete, sign and date the Fingerprint Background Waiver prior to having your fingerprints taken. The Fingerprint Background Waiver has to pre-date the date on your fingerprint card.
   
   
2. Please include the Fingerprint Background Waiver with your licensing application packet and a cashier's check or certified check made out to the Department of Public Safety with the correct amount.  All personal checks will be returned to you as the Department of Public Safety does not accept personal checks.
